takes your personal privacy seriously. CareAmerica Life is committed to

Nonpublic Personal Information
CareAmerica Life Collects nonpublic personal information (such as the
policyholder or insured person’s name, address, date and location
of birth, marital status, sex, social security number, medical information
and beneficiary information) on enrollment applications, evidence of insurability
forms and claim or proof of loss forms.
Use of Nonpublic Personal Information
CareAmerica Life uses nonpublic personal information to underwrite policies,
process claims, ensure proper billing, administer and service benefits
as well as offer other insurance products that may suit additional needs
of our policyholders and insured persons.
Nonpublic Personal Information Disclosure:
- CareAmerica Life may provide nonpublic personal information to an affiliated
or nonaffiliated person or organization involved in the underwriting,
processing, or servicing of our products. CareAmerica Life will not provide
nonpublic personal information to any other nonaffiliated person or organization
without a written agreement requiring protection and confidentiality of
such information.
- CareAmerica Life may have to provide nonpublic personal information
to local, state and federal authorities in accordance with examinations,
subpoenas or summons as required by law.
Security of Your Information
CareAmerica Life has established policies to maintain physical, electronic,
and procedural safeguards to maintain the confidentiality of the nonpublic
personal information of our policyholders and insureds. Appropriate measures
are taken to ensure that access is available only to those individuals
who need to know that information in order to provide our products or
services.
